JAIL FORMS
The purpose of these suggested jail forms is to provide guidance regarding the minimum information that should be collected about an inmate at the time of intake/booking and the minimum healthcare information that should accompany an inmate upon transfer to another jail, prison or other treatment facility. Jails may choose to use these forms as provided or may use them as a guide in the development or refinement of their own forms. Each jail should also develop corresponding operating policies and procedures regarding the use of these forms or similar forms used in their operations.
Correctional Mental Health Screen (CHMS) Tools:
These forms have been developed and reviewed by an assortment of practitioners in the field of corrections. They have not been reviewed by any legal authority. Any legal questions an agency may have should be reviewed by the agency’s legal advisor.
